GENERAL INFORMATION REQUEST FORM
Customers may use this form to request general information from the District, as well as a plant tour, or inquire about outreach event opportunities.
NON-URGENT SERVICE REQUEST FORM
Residents may use this form to report non-urgent incidents or concerns for potential District field inspection; i.e. a parked car that is leaking motor oil which may drain into a nearby storm drain or creek. If unsure or need immediate attention, please contact us at 707-429-8930.
UTILITY INFORMATION REQUEST FORM
Engineers, developers or public agencies may use this form to request District utility information. After filling up the form, you may attach drawings or create a simple map delineating the general scope of work.
